My tests are normal, so why am I in pain?

This text is awaiting the doctor's approval and may change.

A normal result is good news, but it does not always rule out a rheumatic disease. The reverse is true as well: a positive result alone is not a diagnosis.

Many patients arrive with a test report, relieved or worried: “Everything is normal, but I still hurt.” That needs to be taken seriously.

What a blood test does not show

Rheumatology tests are tools, not verdicts. No single test proves or rules out rheumatoid arthritis. Not everyone with the disease has a positive RF, and some people have it with completely normal blood tests. Anti-CCP is found in only about 60 to 70 percent of patients.

In some diseases this is even clearer. In spondylitis and psoriatic arthritis, the main clue usually comes from the examination and imaging, not from a number in the blood.

Images do not always answer either

X-rays are usually normal in early arthritis, because the bone has not yet been damaged. Ultrasound and MRI may show inflammation sooner.

So where does the diagnosis come from?

From putting these together:

  • Your story: when and where the pain began, what it is like, and what mornings are like
  • The examination, which is often the most important part of the visit
  • The test results and images

Sometimes the full picture becomes clear only with time, and your doctor asks you to come back after a while.

The reverse is true as well

A positive result does not by itself mean disease. Some people have a positive RF and never become ill, or have another disease. So do not panic over one abnormal number either.

It is not always rheumatism

Joint pain has many causes. Joint wear, tendon injury and other diseases also cause pain. That is another job of the visit: ruling out other causes so the right treatment can be chosen.

What should you do?

If your joints have been swollen and stiff for several weeks, or the pain comes with long morning stiffness, mouth ulcers, rashes or a red eye, do not take a normal result as a reason to stay away. Note your symptoms and bring all your earlier results.
